Building Use Form: This form is to be filled out be persons or organizations who are requesting the use of Fair Haven High School facilities for non-school related functions. Please return the completed form to the activities coordinator, Jen Lopez, for processing. pdf icon
Field Trip / Bus Request: This form is to be filled out by staff members requesting to take students on a field trip and/or to use school buses for a trip. Please return the completed form to the activities coordinator for processing. pdf icon
Parent Transportation Request: This form is to be filled out by parents who wish to arrange to transport their own children home from a school event to which the student was initially transported by school bus.
